Appointments

Appointments may be scheduled online, by phone, or in person. Availability, services, and pricing are confirmed by the clinic at the time of scheduling and may change. If you are unable to attend your appointment, we kindly request at least 24 hours’ notice whenever possible. Repeated missed appointments without notice may affect future scheduling.

Payment & Insurance

Payment is due at the time services are provided unless other arrangements have been made. We accept most major insurance providers; however, insurance coverage varies by individual policy. Patients are responsible for any deductibles, co-payments, non-covered services, or remaining balances not paid by their insurance provider.
